FIRST TIME TO ONE OF MY CLASSES?

If it’s your first time, then why don’t you download the two forms you need to fill out to save you time on the day? You need to print and sign the following:

1. PARQ Form – This lets me know about your health/medical status
2. Informed Consent Form – To confirm that you know what you’re letting yourself in for!
