You must fill out this form if it this is your first time accessing an online class with Warwickshire Fitness.

Many health benefits are associated with regular exercise, and the completion of the Par-Q is a sensible first step to take if you are planning to increase the amount of physical activity in you life. For most people physical activity should not pose a problem or hazard.

The Par-Q has been designed to identify the small number of people for whom physical activity might be inappropriate or those who should take medical advice concerning the type of activity most suitable for them. Common sense is your best guide for answering these few questions, Please read them carefully and tick the Yes/No box opposite the question as it applies to you.

IMPORTANT: If you answer yes: If you answer yes to one or more questions and have been inactive or are concerned about your health, you must consult a physician before taking a fitness class or substantially increasing your physical activity. You should ask for a medical clearance along with information about specific exercise limitations you may have. In most cases, you will still be able to do any type of activity you want as long as you adhere to some guidelines. Warwickshire Fitness reserves the right to request a Medical Clearance certificate from your GP before permitting you to take part.

If you answer no: If you answer no to all the PAR-Q questions, you can be reasonably sure that you can exercise safely and have a low risk of having any medical complications from exercise. It is still important to start slowly and increase gradually. It may also be helpful to have a fitness assessment with a personal trainer or coach in order to determine where to begin.

Your Data

The information you have given on the PAR-Q form is required for the following reasons: • to make sure that in the event of a medical emergency, the correct First Aid or action can be administered
• to ensure that any medical conditions you have declared are covered by our insurance
• to determine whether you are above the age of consent and are covered by our insurance

You may request a copy of the information we hold about you. Likewise you may request for us to delete all data held about you. Please note that in this case this may prevent you taking part in fitness classes or Personal Training sessions with Warwickshire Fitness from that point on.

We will never pass your personal information or data to third parties.

I understand that my data will be used solely by Warwickshire Fitness and that I am able to review this data at any time and ask for it to be deleted.

Programme objectives and procedures
I understand that the purpose of this exercise programme is to provide safe and individualised exercise to improve health and fitness. Exercise may include:
• Cardio-vascular activities – Aerobic type activities such as rhythmical movement or dance to music
• Resistance training activities using body weight and light resistance equipment such as bands to improve muscular strength and/or endurance
• Flexibility exercises to improve movement around the joints and range of motion

Potential risks
The exercise programme is designed to place a gradually increasing workload on the cardio-vascular and muscular systems and thereby improve their function. The reaction of the cardio-vascular and muscular systems to such exercise cannot always be predicted with complete accuracy. There is a risk of certain changes that might occur during or following the exercise. These changes could relate to blood pressure or heart rate.

I understand that I am responsible for monitoring my own condition throughout the exercise programme and that should any unusual symptoms occur, I will cease my participation and inform the instructor of the symptoms.

Potential benefits
I understand that a programme of regular exercise has been shown to be beneficial. Some of these benefits include; a decrease in risk of heart disease, a decrease in body fat, improved blood pressure, improvement in psychological function and aerobic fitness.

The exercise programme has been explained to me and my questions regarding the programme have been answered to my satisfaction. I understand that I am free to withdraw at any time. The information obtained will be treated as private and confidential.
